*** To be considered as having completed registration you must have the packet completed, and necessary forms turned in and signed off by the Registrar. This includes the correct grade verification, birth certificate, and physical, as well as address verification is needed. This information can be uploaded via your registration account, emailed reg@galtjrwarriorsyouthfootballandcheer.com or handed in at a practice or drop off day TBD to the registrar. ***Proof of residence ( ONLY needed if from school that is out of boundaries) - this can be a copy of mortgage, or utilities document must have name of parent and address in question
***Physical forms must include the wording cleared for sports, cleared for athletic activity, or the box marked "SPORT PARTICIPATION APPROVED" on the MVFL physical form or associated doctors physical form. It also must include the doctors signature, written or digital, with the date.
***Proof of grade must be from current or prior year grade level, If not from school in boundaries must provide proof of residence in organizations boundaries.
